What information is required on the Record of Motor Fuel Used - COM-RAD 061?

Record of Motor Fuel Used - COM-RAD 061 is a record of the gallons of motor fuel that were used in your vehicles and/or equipment.  You must report the type of equipment and/or vehicle identification number, type of fuel purchased, and the daily or monthly gallonage totals.  These totals should match the totals on your refund claim form.
